April 5, 2006: A pilot was critically injured when he had to eject during a training mission over the ocean, about 80 miles northeast of Charleston. An investigation found the pilot blacked out after a high-gravity maneuver while traveling at 750 mph.
April 18, 2005: A two-seat F-16 crashed into the Ashley River near downtown Charleston. Both pilots ejected and escaped serious injury. Investigators said both main power and backup power sources failed before the mishap.
March 9, 2004: Two fighters collided in midair during a training mission over the ocean near Myrtle Beach. Both planes returned to Shaw and neither pilot was injured. An investigation blamed pilot error for the mishap, which caused more than $200,000 damage to the planes.
March 19, 2000: Maj. Brison Phillips died when his plane crashed during an air show near Kingsville, Texas. Phillips was a member of the F-16 demonstration team.
